一、參加資格：具備中華民國國籍，且目前就讀台灣或離島國中以上之先天顱顏患者。
(齒顎咬合不正、血管瘤之患者不包含在內。)
二、受理收件日期：自 114 年 8 月 4 日至 114 年 9 月 12 日止，由基金會活動入口網站線上或紙本郵寄，以郵戳為憑。
三、獎項類別：每年度只能選擇一類，不得重複申請。
